What to Expect at Your First Visit
Your initial appointment is designed to build a strong foundation for your ongoing care. Here’s a typical flow:
- Check-In & Paperwork
Arrive 15-20 minutes early to complete new patient forms (or access them via the Patient Portal). We’ll collect your insurance details, photo ID, and basic information. - Vital Signs & Intake
One of our friendly medical assistants will take your vitals (weight, blood pressure, temperature, pulse, etc.) and review any immediate concerns. - Meeting Your Provider
You’ll meet one of our board-certified physicians or a nurse practitioner. Our lower patient load means more time with your provider – you won’t go more than two visits without seeing your doctor! Expect a thorough discussion of your medical history, current medications, symptoms, lifestyle, family history, and goals. For our geriatric patients, we will take time to address your medication management, mobility, cognitive health, and any preventive needs. - Diagnostics & Testing, if needed
We may perform on-site diagnostics like EKG, spirometry, or basic lab tests if needed. All labs are performed right in your exam room for your comfort and convenience. Once our Imaging services are open, we will perform any x-rays. - Personalized Plan
We’ll create a customized care plan, discuss preventive screenings, chronic disease management, and/or referrals. You’ll leave with clear next steps and access to our patient portal for ongoing communication.
What to Bring
To make your visit smooth, bring your:
- Photo ID and insurance card(s)
- List of current medications (including doses and supplements)
- Recent lab/imaging results or records from other providers
- List of past surgeries, hospitalizations, allergies, and chronic conditions
- Any symptom notes, questions, or concerns
- For geriatric patients: Caregiver details, advance directives, medical power or attorney, and/or fall history, if relevant

Patient Responsibilities
- Provide your current insurance information prior to, or at the time of, your visit (we verify benefits to help you understand coverage and financial responsibility).
- Understand your benefits, including copayments, deductibles, and coinsurance—our staff is happy to assist.
- Bring your insurance card and photo ID to every visit.
- Copayments are due at the time of service.
- Deductibles and coinsurance will be billed after insurance processing; outstanding balances must be paid before or at future visits.
- For self-pay patients, payment in full is expected at the time of service (ask about self-pay options or payment plans).
- Non-covered services are the patient’s responsibility—we can help verify benefits in advance.
- Some plans require referrals or prior authorizations—please check with your insurance.

No-Show & Cancellation Policy
We’re flexible and happy to work with your schedule to keep your medical care convenient. Since we value everyone’s time and aim to provide timely access to care for all our patients, we have the following policies:
- Cancellations: Please cancel at least 24 hours in advance. Late cancellations (under 24 hours) may incur fees—we’re flexible and happy to work with you.
- No-Shows: You appointment may be rescheduled, or counted as a no-show by failing to arrive without notice, or arriving more than 15 minutes late.
- Fees: $25 no-show/late cancellation fee (patient responsibility, not billed to insurance).
- Repeated Issues: 2+ no-shows and/or late cancellations within 12 months may require pre-payment or lead to dismissal (per policy and law).

Medical Paperwork & Forms Fee Schedule
ApexMed Primary Care is a paperless medical office and charge a fee for print copies of completed forms (e.g., disability, FMLA, insurance verification) as follows:
- Disability/FMLA/Insurance Forms: 7-10 business days, fees vary
- Medical Records Copy: First 10 pages free, $0.50 per page after
- Work/School Physicals: $10 (based on recent exam)
- Detailed Medical Letters: $25
- Immunization Records: $10
- Legal/Attorney Requests: $50-$100
- Expedited (under 48 hours): Additional $25 rush fee
