These guides are written by our primary care team for adults — especially patients 65 and older, caregivers, and anyone managing a long-term condition. The goal is the same one we use in clinic: quality of life, independence, and evidence-based care. Not hype.

How to Use These Guides

  1. Read the piece that matches your next question — a screening, a new diagnosis, a plan for aging well, or a hard season.
  2. Write down one or two questions for your provider.
  3. Bring your full medication list, including supplements and over-the-counter products.
  4. If you need care, not just reading, call us. Appointments are scheduled by phone so we can match you with the right visit.
